He was predeceased by: his parents, Stephen Berleth and Helen Berleth (Kaurich); and his siblings, Steven Berleth and Helen Dabule. He is survived by: his daughters, Stephanie B. Khosla (Vineet) of Rockville Centre, NY, Doral Lysiak (Michael J.) of Johnstown, NY and Erica S. Berleth (Neil McManus) of North Tonawanda, NY; his grandchildren, Nikhil, Meghan, Deven, Lauren, Samantha, Allison, Sean and Aiden; and his sister Mary Helen Virkler. He is also survived by extended family and friends.
