He was predeceased by: his parents, George Everett and Hazel Jack (McCabe); and his sister Eileen Kelley. He is survived by: his wife Dorothy M. Jack (Reeder); his daughter Bonnie Cunningham (George) of Louisville; and his grandson Gregory Cunningham (Karen).
