He is survived by: his parents, Sandra Reeves Collins and Marc Cook of Athens; his daughters, Kyleigh Collins and Samantha Collins of Pickerington, Ohio; his sisters, Jade Collins and Jessica Jordan; and his stepsister Heather Stack. He is also survived by several aunts, uncles, nieces, nephews and friends.
