He was predeceased by: his parents, Earl Blake and Della Blake (Hann); his wife Dorothy Siniff Blake; his children, Terry L. Blake and Marjorie Krementz; and his siblings, Scott Blake and Marcile Greenawalt.

He is survived by: his children, Irena Siefker (Kenneth) of Ottawa, Paul Blake (Jackie) of Corona, Michael Blake of Celina and Barb Clum (Bryan) of Lima; and his daughter-in-law Irene Blake of Lima. He is also survived by numerous grandchildren, great-grandchildren and great-great grandchildren.

Memorial contributions may be made to St. Mary's Catholic Church.
