He was predeceased by: his parents, Lincoln Mcintosh and Opal Mcintosh; and his siblings, Dorothy Mcintosh, Dennis, Wayne, Cletus, Ray and Bobby Mcintosh.

He is survived by: his wife Cassie Jenkins; his grandchildren, Jeffrey Allen Gullett, Jeffrey Gullett (Shanna Mustard) and Claudie Gullett (Ashley); his great grandchildren, Jacob Gullett, Cadence Mustard-Gullett and Aysen Mustard-Gullett; and his sisters, Berta Mitchell and Betty Webb.
