He was predeceased by: his sister Amy Polk. He is survived by: his parents, Richard Polk of Columbus and Joan Smith Schwimer; his siblings, Gretchen Furlong (Jim) of Lancaster, Megan Dennard (Paul) and Stephen Dysert (Andi) of Buffalo; and his nephews, Drew Doherty and Seamus Furlong.
