It is with great sadness that we announce the death of Guy Omer Forrest III (Columbus, Ohio), who passed away on February 4, 2019, at the age of 66, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his grandchildren, Shawn and Lilly; and his brother James. He is survived by: his wife Geri; his children, Craig Forrest (Cassie), Christina, Matthew, Heather Adams (Andy) and Mark Fausnaugh; and his grandchildren, Christian, Tori, Frazier and Claire.

In lieu of flowers, donations may be made to the Epilepsy Alliance of Ohio.
