He was predeceased by: his mother Garnet Toole Frase. He is survived by: his wife Jean; his son Chris (Julie); his grandchildren, Nathan (Desiree), Jordan (Alex) and Miranda (Kyle); his great grandchild Adam; his brothers, David (Debbie) of Texas and Joseph (Nancy) of Dayton; and his sister-in-law Opal Keirns. He is also survived by Nieces, nephews, cousins, many friends.
In lieu of flowers memorial contributions may be directed to Mt. Carmel Hospice.
