He was predeceased by: his father George Hazlett; and his grandparents, Margaret Hazlett and Patrick Collins. He is survived by: his mother Tammy Collins; his siblings, Keith Collins, Anthony Collins, Lucky Collins, Cody Hazlett and Leshelle Collins; and his grandparents, Rose Collins and Roger Hazlett. He is also survived by many aunts, uncles, nieces, nephews, cousins and friends.
