She was predeceased by: her husband Steve; and her sister Betty VanHorn. She is survived by: her daughter Gale Smith (Tom); her grandson Mark Smith (Moriah); her great grandchildren, Mark Jr., Nicole, Leah and Katy Smith; and her nieces, Janet Crooks and Robin Wall of Tennessee.

Memorials may be made in Phyllis's name to Windsor House at Doylestown Health Care Center & Crossroads Hospice and Palliative Care.
