He was predeceased by: his daughter Diane Jo Sullivan; and his parents, Joseph Sullivan and Thelma Sullivan. He is survived by: his wife Nancy; his children, Debra Summerfield, Stephanie McClean (Jason), Leslie Urteaga (Raphael) and Thomas Jr. (Darla); his grandchildren, Liam McClean, Vincent, Jacob Urteaga, Brandi Walls and Nikki Summerfield; his great grandchildren, Shianne, Savannah, Blake, Cody and Kohle; and his great great grandchild Arianna.
