He was predeceased by: his parents, William F. Herbert and Elizabeth A. Herbert; and his siblings, Glendon, Virgil and Wanda.
He is survived by: his wife Lois Janet Johnson; his daughters, Melissa McCray (Bobby) and Stephanie Smith (Dennis); and his grandchildren, Justin, Brandon, Mikayla and Aidan. He is also survived by numerous nieces and nephews.
