She is survived by: her parents, Leroy Havens and Mary Sue Havens (Hornsby); her caregiver DaVita; her children, Alexis, Jessica and Matthew Crutchleo; and her siblings, Monica Hilinski (Tim) and Brian Havens (Sherry). She is also survived by numerous nieces, nephews, cousins, other relatives and friends.
