He was predeceased by: his parents, Johnny Marvin and Charlie Mae Cook (Ward); and his siblings, Leonard Cook, Dollie Cesario, Johnnie Shine and Barbara Pitts.
He is survived by: his stepmother Harriett Cook of Lima; his son Shawn Cook of Lima; his siblings, Michael Cook (Renee) of Georgia and Debra Cook Tibbs of Lima; and his brothers-in-law, Mark Pitts of Lima and Eddie Shine of Lima. He is also survived by nieces, nephews, other relatives and friends.
