He was predeceased by: his daughter Lynette Fuqua. He is survived by: his wife Veronia; his children, Levolia O. Fuqua, Sr. of Columbus, OH., Monique Fuqua-Cummings (Kareem) of Columbus, OH and Annette Fuqua of Lima; and his siblings, Barbara Peters of Lima, Howard Fuqua, Jr. (Sheron) of Killeen, TX and Jerome Fuqua of Lima. He is also survived by nieces, nephews, other relatives and friends.
