He was predeceased by: his parents, Lewis A. Sr. Cullen and Viola Cullen (Sullivan); his wife Betty J. Cullen (Stumbaugh); his daughter Debra Jean Cullen; and his sister Karen Perrine.
He is survived by: his son David Cullen of Van Wert; his sister Virginia Woods; and his sister-in-law Janice Bigelow. He is also survived by three grandsons; one granddaughter; and ten great-grandchildren.
