Cook of Lima; his children, Deon Cook (Lawrence "Nick"), Price of Hilliard, Roy Maxwell Cook IV of Venice, FL and Troy E. Cook; and his brothers, Correll "Don" Cook (Pam) and Dave Cook of Columbus. He is also survived by fourteen grandchildren, nine great-grandchildren.
