He is survived by: his mother Lynn Jacobs; his stepfather Mike Jacobs; his children, Dalton Matz, Gracie Matz and Leigha Matz; and his siblings, Cody Matz (Shannon) of Mansfield, Jeremy Matz of Loudonville and Stephanie Matz.
