He is survived by : his parents, Charles Dee Puckett of Lucasville and Betty Jo Smith of Minford; his children, Jeffrey Puckett of Columbus and Britany Puckett of Columbus; his siblings, Jerry Lee Puckett (Bobbie Jo) of Portsmouth, Kathy "Kitty" Crabtree (Tony) of Lucasville, Penny Puckett (Rick Carver) of Lucasville and Chelsey Heinlen (John) of Lucasville; and his nieces and nephews, Ryan Crabtree, Tiffany Chidsey, Ayla Puckett, Shawn Puckett, Mark Cox, Ashley Puckett, Kylee Lepore, Elizabeth Heinlen, Sofia Heinlen, Jennifer Hall, Rebecca Crabtree, Tony Crabtree, Jr. and Hannah Hamilton. He is also survived by numerous Uncles, Aunts, Cousins.
