We are sad to announce that on July 25, 2021, at the age of 75, Jack Kaminski of Middlefield, Ohio, born in Chardon, Ohio passed away. Family and friends are welcome to send flowers or leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Stanley Kaminski and Mary Elizabeth Kaminski (Adams); and his granddaughter Teddie Lynn Chudzik. He is survived by: his wife Barb Herakovic; his children, Angel Chudzik (Ted) of Orwell and Jack Kaminski (Shelby) of Windsor; his grandchildren, Lucas Varner, Alex Chudzik, Eve, Ivy, Jessman, Nora Jean, Jett and Zebulon Kaminski; his sister Joanne Hopes of Orwell; and his brother-in-law Mike Herakovic of Middlefield. He is also survived by several nieces and nephews.
