He was predeceased by: his parents, William Smith and Emily Smith (Bond); his sons, Tommy and Brian; and his sister Dorothy McCormick.
He is survived by: his wife Colleen Smith; his children, Rich Smith, Andrea Holt and Chris O'Shell (Michelle); his grandchildren, Rikki, Rhiannon, Brianna, Stephanie, Corey, Shawna, Maria, Samantha and Michaela; his brother Tom Smith (Barb); his brother-in-law Bill McCormick; his pet Baxter; and his friends, Tina, Jennifer and Kyle. He is also survived by numerous nieces, nephews & other relatives and friends.
In lieu of flowers, memorial donations may be made to Ohio's Hospice of Dayton or Vancrest of New Carlisle.
