He was predeceased by: his parents, H. Weir Cook, Jr. and Margaret Cook (Lenahan); and his sister Margaret Locke (Tom). He is survived by: his wife Peggy Cook; his daughter Colleen Herr (Jason); his grandsons, James and Grant; and his siblings, Joanne Green (Glenn), Kathy Foulk (Larry), Patty Shore (Dave), Thomas (Teresa), Connie TenBricK John Cook (late Larry), Dorothy Mann (David) and Maureen Pinnick (Mike). He is also survived by many nieces and nephews.

In lieu of flowers, donations can be made to Ohio Health Hospice.
