She was predeceased by: her father Henry; her husband Melvin Willoughby; and her children, Paulette Slavings, Henry Simmons and Mary Ann Simmons.
She is survived by: her sons, Joel Simmons (Maggie), John Simmons (Linda) and Donal Simmons (Lorna); and her children-in-law, Timothy Slavings of Rittman and Linda Simmons of Wadsworth. She is also survived by 20 grandchildren; numerous great and great great grandchildren.
