He is survived by: his wife Chris Blake (White); his children, Jeremy Blake, Michelle Shields (Eric) and Erick Blake (Heather); his grandchildren, Shawn Blake, Sadie Vizzuso, Dillon Blake, Tori Shields, Hunter Shields and Avianna Blake; and his siblings, LucyAnn Ziegler, Barb Barringer, Susan Reed, Dave Blake, Sandy Gandee and Nancy Brooks.
