With heavy hearts, we announce the death of Carol Ann Antonishak (Struthers, Ohio), born in Windber, Pennsylvania, who passed away on April 18, 2021 at the age of 71. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was loved and cherished by many people including : her parents, Gino Leonardis and Mary Leonardis; her husband Thomas; her children, Cary Jernigan (Geoffrey) of Littleton, CO and Thomas Antonishak (Tammy) of Poland, OH; her brother Gino Leonardis (Barb) of Windber, PA.; and her grandsons, Colton, Travis, Tommy and Angelo.
In lieu of any flowers, Carol's family requests memorial tributes take the form of contributions to the American Diabetes Association.
