Carolyn Mosko Obituary
It is with great sadness that we announce the death of Carolyn Mosko (Toledo, Ohio), who passed away on May 14, 2020, at the age of 79, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Henry Chmielowicz and Florence Chmielowicz (Kondalski); her husband John; and her siblings-in-law, Deanna Chmielowicz (Jim), Betty Mosko (Joseph) and Stanley Mosko.
She is survived by: her children, Dave Mosko (Linda), Lynn Blakeslee (Don) and Susie Kozal (Andrew); her brothers, John Chmielowicz (Karen) and Jim Chmielowicz; her grandchildren, Monica Ashcroft (Kevin), Steven Mosko, Meghan (Chloe Kozal) and Grant (Kennedy Blakeslee); her great grandchildren, Wyatt and Everett Ashcroft; and her siblings-in-law, Kit Mosko, Mary Jane Czyznik (Ron) and Joseph Mosko. She is also survived by numerous nephews and nieces, great-nephews and great-nieces, and cousins.
In lieu of flowers, the family suggests memorial donations be made to Central Catholic High School, Notre Dame Academy, St. Francis de Sales High School, or the Parkinson's Foundation of Northwest Ohio.
