He was predeceased by: his parents, John Forbes and Louella Mackey Forbes. He is survived by: his wife Melody Joy Egts Forbes; his children, Jack Forbes Jr. (Virginia) and Michelle Anderson (Dwayne Sr.); and his siblings, John Forbes Jr., Brian Forbes Sr. and Patti Sperry (Ted). He is also survived by 7 grandchildren.
