She is survived by: her husband Jack F. Cook; her children, Lesley Cook Jackson, Matthew R. Cook, Sr. (Lisa) and Stacey Cook Babich (Stephen); and her grandchildren, Steven Jackson (Theresa Gue), Greg Jackson (Kenisha), Ryan Jackson (Sarah), Matthew R. Cook, Jr., Tyler Cook (Courtney), Jonah Cook (Meggan), Lacey Cook and Alex Babich (Hope). She is also survived by 2 great-grandchildren.
