She was predeceased by : her great-grandparents, Helen Teegarden, Ray and Goldie Phelps; and her uncle Corey E. Osmon. She is survived by : her mother Brenda J. Hayes (Phelps); her sons, Damien Blaze Osmon and Keanan George Anthony Osmon; her parent Shawn Osmon (Rebecca); her siblings, Taylor Marie Hayes, Timothy John Patrick Hayes and Savannah Osmon; her nephew Justin Edward Bishop Jr.; her grandparents, Susan Kuehner, Timothy R. Phelps (Connie), Michael Kuehner, Linda Starkey (Gary) and Richard Osmon (Sue); and her aunts and uncles, Shannon Garner (Adam), Jennifer Schmus (Rick), Patrick C. Hayes II, Jeremy Hayes, Adam Hayes (Christina) and Anthony Hayes Sr. (Holly). She is also survived by several aunts, uncles, cousins and friends.
