He was predeceased by : his parents, Eskel Blair and Kate Williams Blair; his wife Judith Ann Jones; his siblings, Sue Anderson, Bill, Ray and Bert Blair; his brother-in-law Harold Jones; and his daughter-in-law Tracy Blair.

He is survived by : his sons, Jack Jr. Blair (Tammy) of Washington, C. H., Robert Blair (Teresa) of Washington, C. H. and Steven Blair of Columbus; his grandchildren, Jeremy Blair (Amber), Larry Justice, Jessica Kuhn (Emmitt), Jacqueline Blair (Harvey), Kayla Knisley (Brent), Steven Blair (Madeline) and Zach Blair; and his aunt Alice "Peach" Ackley. He is also survived by seven step-grandchildren, eleven step-great grandchildren.

In lieu of flowers, the family asks that donations in Jack's name be made to one or both of the following organizations: Hospice of Adena Health System, 2077 Western Avenue, Chillicothe, Ohio 45601 and/or the Fayette County Commission on Aging, 1179 S. Elm Street, Washington C.H., Ohio 43160.
