He was predeceased by: his parents, Ralph M Church and Marguerite E Barnes Church; his brothers, William Church, Harry Church and Walter Church; and his daughter-in-law Rita McFerren Church.
He is survived by: his wife Sandy Hoffer Church; his sons, Carl Church and Michael Church; his grandchildren, Dallas Church and Paige Church; his great grandchildren, Owen Church, Garrett Church and Nevaeh Church; and his sister-in-law Diane Kreis. He is also survived by many nieces and nephews.
