He is survived by: his wife Sandra Cook; his children, Jennifer Brown (Zack) of Checotah, OK and Douglas Cook (Jayme Smith) of Broken Arrow, OK; his sister Barbara Hughes of Sapulpa, OK; his grandchildren, Corbin Cook, Angelina Smith and Rosemary Peppers; his great grandchildren, Paisleigh Smith and Madilynn Smith; and his pet Molly. He is also survived by many relatives and friends.
