He is survived by: his parents, Kenneth Leon Middleton and Phyllis Irene Henderson Middleton; his daughter Felicia Mae Mendoza of Catoosa, Oklahoma; his stepdaughter Mary Lois Middleton of Florida; his grandchildren, Christopher Lee Miller, Tamacelo Velazquez and Elizabeth Velazquez; and his stepgrandchildren, Mark Johnson, Jr., Cloe Johnson, Kassandra McGuire, Temprance McGuire, Matthew McGuire and Thomas McGuire.
