He was predeceased by : his grandparents, Clyde, Ethel Bellamy, Edgar and Naomi George; his father John Cox Bellamy; and his brothers, Jackie Dale Bellamy and Johnny Dean Bellamy.

He is survived by : his mother Mary Ellen Bellamy (George); his wife Frankie Joe; his children, Heath Hull and Michelle Plaster (Jimmy); his siblings, Bill Bellamy (Joyce), Marilyn Cook and Susie White (Louie); his grandchildren, Jade Ellis (John), Zack Plaster, Bethany, Cyndie Scott (Carter) and Blake Plaster; and his great grandchildren, Jet Ellis and Letty Ellis.

In lieu of flowers, the family has requested that memorial donations be made to the Oklahoma Medical research Foundation, 825 NE 13th Street, Oklahoma City, OK 73104.
