He is survived by: his wife Evelyn Lewis (Cruts); his children, Donna Jo Kliewer of Allen and Michael Alan Lewis of Wakita; and his siblings, JoAnne Cummings, Rosalee Warner and Roger Lewis. He is also survived by five grandchildren and numerous great grandchildren.
