He was predeceased by: his wife Shirley Cook; his daughter Jody Cook; his parents, Bonnie Cook and C. H. Cook; and his siblings, Kenny Cook, Butch Cook and Juanita Whiting.
He is survived by: his children, Teresa, Alvan Walker of McAlester, Brandi, Eddie Mullins of McAlester, Butch Cook of McAlester, Jimmy Cook, Regina Scott of Krebs and Marc Strain of California; and his sisters-in-law, Pybles Cook of Adamson and Sue Cook of Hartshorne. He is also survived by 10 grandchildren; 20 great-grandchildren; brother.
