He is survived by: his former spouse Lindsay; his wife Martha; his stepdaughter Megan Leone (Francesco) of Milan, Italy; his siblings, Beverly (Ralph Evans) of Enid, Joseph Blake (Sally) of Oklahoma City, John Blake (Susanne) of Pauls Valley and Stephen Blake (Sandra) of Oklahoma City; his siblings' children, Mary Ragna Evans Yetter, Blake Evans, Paul Evans, Jeff Blake, Shannon Blake Walker, Stephanie Blake Russell, Chris Blake, Boston Blake, Savannah Blake Cooper, John Trubey and Dawn Agnew; and his close friends, Jordan Humphrey and Tori Agee.
In lieu of flowers, contributions may be made to the American Diabetes Association.
