He was predeceased by: his parents, Joe Lewis and Hazel Osborn Lewis; his wife Jackie Lewis; his daughter Johnnie Lewis; his brother Jeff Lewis; and his great grandchild Tatum.

He is survived by: his former wife Mary Lou Lewis; his children, Jay, Judy of Sasakwa, OK, Bo and Michelle Burk of Ada, OK; and his grandchildren, Keith Lewis, Jamie Grant, Kendra Hunt, Jayson Lewis and Keaton Scott.
