He was predeceased by: his parents, Roy A. Fornwalt and Helen M. Fornwalt (Farabaugh); and his cousin Kathy Mika. He is survived by: his wife Donna M. Curry; his sister-in-law Debra Shaffer (Robert); his sibling's children, Sandra Marie Herrera and Scott Eliot Shaffer; his cousins, Kenneth M. Farabaugh, Jr., Robert Farabaugh, Mary Jo Leamer, Annette Almes and Lori D'Amato; and his pet Teddi. He is also survived by great-nieces; great-nephews.
Visitation will be held on Saturday, October 25th 2025 from 9:00 AM to 11:00 AM at the St. John the Evangelist Catholic Church (344 Baynton Ave, Altoona, PA 16602). A funeral Mass will be held on Saturday, October 25th 2025 at 11:00 AM at the same location.
