He was predeceased by: his parents, Esther Spak (Neary) and Daniel Spak; and his siblings, Myron, John, Mary Korzeniewski and Adrianne Iskra.
He is survived by: his wife Arlene Spak (Severnak); his daughter Kimberly Blazejewski (David Blazejewski); his grandchildren, Sara Blazejewski of Philadelphia and Daniel Blazejewski; his brother Joseph Spak of Wilkes-Barre; and his siblings-in-law, Jack Severnak of West Wyoming and Sylvia Severnak of Pittston. He is also survived by numerous nieces and nephews.
In lieu of flowers, the family lovingly asks that memorials be sent to the Gary Sinise Foundation.
