He was predeceased by: his parents, Burt Hazzard and Edith Hazzard (Anderson); his wife Catherine B. Hazzard (Holes); his son-in-law Charles L. Laubham; and his grandson Charles Laubham.
He is survived by: his children, Marian Laubham of Bradford and Donald Hazzard (Laurie) of Ritterville; his grandchildren, Tammy Lea Gardner, Daniel Hazzard, Thomas Hazzard and Josh Kinney; and his great grandchildren, Katie Theim and Hunter Hazzard.
In lieu of flowers, memorial contributions may be made to Friendship Table East Corydon St. Bradford, PA, or a charity of the donor's choice.
