Perry (Jessica) of Carbondale, PA and Louisa Jane Polach (Paul Salvatore) of Carbondale, PA; her grandchildren, Noelle, Scott, Stephen and Samantha; her great grandchildren, Trevor, Jessica, Benjamin and Bruce; her siblings, Hetty Engel of Holland, Leen of Holland, Kees van of Holland, James van and Blink of British Columbia, Canada; and her siblings-in-law, Margaret Brown of Ontario, Canada and Norman Perry of Ontario, Canada. She is also survived by several nieces and nephews.
