She was predeceased by: her parents, Lester W. Stephenson and Rhea G. Stephenson (Weible); her son Eric; and her siblings, Patricia Salisbury, Donna Stephenson and Lester L..
She is survived by: her children, Frank A. "Tony" (Charlene), Mark Alan and Leslie Sample (Joseph); her grandchildren, Stephanie Marie, Alyssa Renee Trimeloni, Janel Sierra Sample (Chad Burns) and Tara Jo Jacobs (Eli); her great grandchildren, Kennedy Jane and Everette Scott Jacobs; and her close friends, Suzie Marko, Kathy Bailey, Bill and Dolly McTonic. She is also survived by numerous nieces and nephews.
Friends who wish may make donations to the cystic fibrosis foundation in recognition of Janice's granddaughters.
