He was predeceased by: his parents, Howard Edward and Margaret Oppman Forsythe; and his siblings, Lois Forsythe Olinger (Roy) and Howard Edward "Jack" Forsythe, Jr. (Ada). He is survived by: his wife Judith Parr Forsythe; his daughter Jeanine Forsythe of Washington, DC; his pet Sophie; his sister JoAnn Hoover (Dick) of Bradford, PA; and his brother-in-law Van Humbert of Connellsville. He is also survived by several nieces and nephews.

Visitation was held on Saturday, September 20th 2025 from 10:00 AM to 12:00 PM at the Brooks Funeral Home - Connellsville (111 E Green St, Connellsville, PA 15425).
