He was predeceased by: his parent John Kopacz. He is survived by: his parent Lucy Rose Kopacz; his wife Jennie Valick-Kopacz; his sister Rose Ann Szychowski (Gerald) of Arizona; and his nieces, Michelle Han (Ken) and Holly Curtis (Dana).

In lieu of flowers, contributions may be made to the Back Mountain Bloomers, P.O. Box 210, Dallas, PA 18612, to dedicate a bench in memory of John (Jack) Kopacz on the Mile 7 of the Back Mountain Anthracite Trail.
