He was predeceased by : his parents, Reamer D. Polk and Viola Polk (Corneilson); his wife May B. Vanderslice; and his brothers, Clayton Polk and Roland Polk.
He is survived by : his sister Mary Martz of Berwick; and his niece Marqueen Girton of Bloomsburg.
A funeral service was held on Monday, January 18th 2021 at 2:00 PM at the Elan Memorial Park (5595 Old Berwick Rd, Bloomsburg, PA 17815).
