He is survived by: his wife Janet L. Costlow; his children, Debbie Dobrowolsky (David) of Blue Knob, Susan Feathers (Kerry), Dennis (Rhonda) of Duncansville, Michael (Lorrie) and Jack Jr. (Julie); and his sisters, Janet Capper of Corry and Sally Hippensteel.
