She was predeceased by: her parents, Joseph B. Nicholas and Violet Bernardini Nicholas; and her siblings, Basil Nicholas and Frances Esposito. She is survived by: her husband Harry C. Fornalczyk; her children, Janice Fullmer (Gary) of Eden, UT and Dr. Robert Fornalczyk (Heidi) of Gibsonia, PA; her grandchildren, Adam Fornalczyk of Cincinnati and Diana Fornalczyk of Cleveland; her stepgranddaughters, Christine Bruce of Louisville, CO and Caroline Fullmer of Louisville, CO; and her great grandchildren, Elise Bruce, Jackson Bruce and Gordon Sjodin. She is also survived by many nieces and nephews.
