He was predeceased by: his parents, Jacob Peruka and Helen Noel Peruka. He is survived by: his sister Jacqueline Smith of Daytona Beach; his nephew Gerald C. Smith Jr. of Palatka, FL; and his great-nephews, Gerald C Smith III and Tyler J Smith.
