He was predeceased by: his parents, Antonio Fazzolari and Maria Fazzolari; and his sisters, Rita, Nunzia and Isabella Fazzolari.
He is survived by: his wife Maria Gulluni; his daughters, Maria Hamilton (Kevin Black) of Hershey, Rita Marcocci (Chris) of Westport, CT and Lisa Fazzolari (Kurt Biehl) of Oakland, CA; his grandchildren, Nick Hamilton and Allie Hamilton; and his siblings, Giuseppe Fazzolari (Brenda) of Harrisburg, Nicodemo Fazzolari (Colette), Salvatore Fazzolari (Karen), Robert Fazzolari (Kelly) of Harrisburg and Rose Belossi (Ferruccio). He is also survived by many nieces and nephews.
In lieu of flowers, the family requests memorial contributions are made in his honor to support educational efforts at St. Joan of Arc School 329 W. Areba Avenue Hershey, PA.
